Form to be used for reporting scores in Tails and Details

This form can be printed out and used to submit trial scores to Tails and Details. Please print, record your information and submit into the Tails and Details box at the club or postal mail to the Tails and Details Editor. You can also download the file, add your information and email the file to editor@potcdogs.com  All scores must be published in the newsletter in the calendar year they were earned in order to be eligible for club awards.

This form can be printed out and used to submit information for titles earned and Club Cups. Please print, record your information and submit into the wooden box at the club or postal mail to the Advisor to the Board. You can also download the file, add your information and email to advisor@potcdogs.com.   These forms must be submitted by December 31 in order to receive awards in January.
